When light hits a transparent surface, it refracts, bending it slightly. Different materials refract the light at different angles. Fiber optic cables are designed such that light inside the cable refracts such that it more or less bounces off the inside of the cable instead of shining out. This allows the light to travel long distances in the cable. Sensors at the other end can detect the light being turned on and off on the other side.
